American Senior Communities is seeking a PRN Occupational Therapist for Hickory Creek Winamac and Hickory Creek Rochester. The OT will provide skilled therapy across ASC settings, evaluate patients, and develop treatment and discharge plans, while supervising COTA and contributing to case management.

The role offers flexible scheduling and the opportunity to work with seasoned therapists and nursing staff to serve senior residents across local ASC facilities.

Qualifications

• Licensed OT with state licensure.
• Master's degree in Occupational Therapy.
• Experience coordinating care and discharge planning preferred.

Responsabilités

• Evaluate patients and provide skilled OT care.
• Develop initial and ongoing treatment plans.
• Develop discharge plans.
• Supervise COTA(s).

The Occupational Therapist implements skilled occupational therapy services for patients in all ASC Therapies & Wellness settings, in accordance with the principles and practices of occupational therapy and within the policies and procedures.

• Evaluate patients and provides quality patient care
• Develop initial and ongoing treatment plans
• Develop discharge plans
• Supervise Occupational Therapy Assistants (COTA)
• Contribute to case management
• Maintain positive level of interaction with patients and center staff
• Adhere to applicable state regulations concerning occupational therapy
