Job summary

American Senior Communities is seeking a PRN Occupational Therapist at Trailpoint Village to deliver skilled therapy to senior residents. You will evaluate patients, create treatment and discharge plans, and supervise COTAs within ASC settings.

Join a team of licensed therapists and Directors of Therapy, collaborating to maintain high standards of care while providing flexible scheduling in South Bend, Mishawaka, and Elkhart areas.

Qualifications

• Evaluate patients and provide quality patient care.
• Develop initial and ongoing treatment plans.
• Develop discharge plans.
• Supervise Occupational Therapy Assistants (COTA).
• Contribute to case management.

The Occupational Therapist implements skilled occupational therapy services for patients in all ASC Therapies & Wellness settings, in accordance with the principles and practices of occupational therapy and within the policies and procedures.
