Job summary

Village Caregiving is seeking a PRN Registered Nurse to join our home care team in Iron Mountain, MI. The RN will visit patients in their homes to complete health assessments, document medical history, and develop personalized care plans for caregivers. Travel is required to cover the Iron Mountain area.

The RN may assist with training new caregivers as needed. This role offers a flexible schedule and is ideal for a nurse who enjoys independent work while making a difference in home health care.

Qualifications

• Graduate of an Accredited Diploma, Associate, or Bachelor’s School of Nursing (or Vocational Nursing).
• Current State RN license.
• Proof of CPR certification.

Responsibilities

• Visit patients in their homes for one-on-one visits.
• Complete initial and ongoing assessments to identify physical and environmental needs.
• Develop a care plan to guide caregivers on how to safely and competently care for patients.
• Educate patients and families on home health care strategies and device recommendations.
• Maintain effective communication with patients, families, the care team, and other health care professionals.
• Complete nurse assessment documents on a supplied tablet and upload to CRM.
• Keep the Director informed about scheduling for upcoming assessments.

Responsibilities

• Visit Patients in Their Homes for One-on-one Visits

• Complete Initial and Ongoing Assessments to Identify the Physical and Environmental Needs of Patients

• Assess Patients and Their Environment to Develop a Care Plan in Order for the Caregiver to Have a Blueprint to Know How to Safely and Competently Take Care of Our Patients

• Educate Patients and Their Families on Proper Home Health Care Strategies and Procedures, and Make Recommendations for Devices or Tools That Might Improve the Patient’s Quality of Life

• Provide Effective Communication to Patients/Clients, Their Family Members, Team Members, and Other Health Care Professionals

• Fill outNurse Assessment Documents on a Supplied Tablet the Same Day As Assessment. Upload Assessment to CRM.

• Keep Timely Communication with the Director on Schedule for Upcoming and Current Assessments.

Qualifications
