Job summary

Frontpoint Health is seeking a Licensed Vocational Nurse (LVN) for PRN/home health visits in Mesquite, Balch Springs, and Forney, TX. You will provide skilled nursing care in patients' homes and coordinate with the care team.

Requirements include 1-3 years LVN experience in adult/geriatric home health, active LVN license (NLC accepted), CPR certification, TB test, and reliable transportation. Kinnser experience is preferred to support care coordination.

Qualifications

• 1-3 years of LVN experience in home health or similar setting.
• Active state LVN license, or multi-state NLC license.
• CPR certification and TB clearance required.

Responsibilities

• Performs skilled nursing services under supervision of a staff LVN.
• Observes, records, and reports changes in patient condition.
• Administers prescribed meds and treatments as allowed by regulations.
• Assists physicians and staff LVN with procedures.
• Assists patients with activities of daily living and self-care.
• Prepares progress notes and adds them to the chart weekly.
• Reports changes to physicians and relevant personnel.
• Participates in in-service education (min. 2 per year).
• Participates in nursing reports and staff conferences.
• Contributes to QAPI plan and process.
