Job summary

Frontpoint Health in Haslet, TX is seeking a licensed LVN for PRN/per-visit home health assignments across patients in their residences. You will coordinate with the interdisciplinary team, families, and referring agencies to administer skilled nursing care within the LVN scope and participate in staffing and planning activities.

The role requires 2–3 years of LVN experience, an active LVN or NLC license, and current CPR, TB, auto insurance, and COVID vaccination.

Qualifications

• Active state LVN license or NLC license.
• 2–3 years LVN experience.
• Good interpersonal nursing skills.
• Proof of CPR, TB, auto insurance, and COVID vaccine.
• Reliable transportation.
• Kinnser experience.

Responsibilities

• Be available Monday–Friday to take patients.
• Perform skilled nursing services under supervision of staff LVN.
• Observe, record, and report patient conditions.
• Administer prescribed medications and treatments as permitted by state regulations.
• Assist physician/staff LVN in specialized procedures.
• Assist patients with daily living activities and encourage self-care.
• Prepare clinical/progress notes and incorporate into the clinical record weekly.
• Report changes to physician and appropriate personnel.
• Perform complex wound care if documented.
• Draw labs by vena-puncture only if competency documented.
• Receive and follow physician orders within LVN practice; do not alter plan of care.
• Participate in in-service meetings and nursing reports.
• Participate in staff conferences regarding patient care.
• Engage in QAPI plan and process.
