Job summary

Frontpoint Health is seeking a Licensed Vocational Nurse (LVN) for PRN/home health in Grapevine, TX. The LVN will deliver skilled nursing care in patients’ homes, coordinate with the interdisciplinary team, families, and referring agencies, and participate in care continuity.

Required are an active LVN license, 1–3 years of LVN experience, current CPR/TB clearance, reliable transportation, and Kinnser experience.

Qualifications

Active LVN license in good standing.
1–3 years of LVN experience required.
Current CPR, TB clearance and auto insurance (if requested).

Responsibilities

Performs skilled nursing services under supervision of staff LVN.
Observes and reports patient condition changes to supervisor.
Administers prescribed medications and treatments as allowed by state regulations.
Assists physician/staff LVN with specialized procedures.
Assists patients with activities of daily living and promotes self-care.
Prepares progress notes and updates clinical records weekly.
Communicates changes to patient’s physician and team.
Performs wound care when documented competency exists.
Performs routine vena-puncture to obtain labs if competency documented.
Follows orders within LVN scope; participates in in-service education.
Participates in nursing report and staff conferences.
Contributes to QAPI plan and process.
