Job summary

Amedisys in Lebanon, TN is seeking a PRN registered nurse to provide weekday home care. You will coordinate all aspects of patient care, develop and implement care plans with the patient care team, educate families on condition and needs, and perform head-to-toe assessments in the home.

Required are an active RN license, current CPR, at least six months home health experience, and a valid driver’s license with reliable transportation.

Responsibilities

• Coordinate and oversee all aspects of patients' home care
• Collaborate with patient care teams to develop and implement care plans

• Perform head-to-toe physical assessments
• Check vital signs and manage medications
• Gives injections, IV therapy or tube feedings
• Administer and maintain catheters
• Complete documentation timely and accurately
• Supervise LPNs and HHAs
• Communicate patient progress to clinical manager and care team

Qualifications

• One year of clinical experience as a registered nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP clinical.*
• Current RN license, specific to the state(s) you are assigned to work.
• Minimum six months home health experience.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
• Able to assess patient status and identify requirements relative to age specific needs.
• Must be familiar with general use and functions of the computer, such as, user names and password concepts; internet; e-mail; navigation of computer desktop, including starting programs, using files and windows, effectively use navigation buttons and tool bars; ability to self-manage online HR services and online training programs.

*Note - If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.
