Job summary

TruHeart Home Health - Norman is seeking a Per Diem Licensed Practical Nurse (LPN) to provide compassionate nursing care in the home setting. As a PRN LPN, you will follow a Plan of Care, monitor patients, assist with activities of daily living, and administer medications as prescribed.

You will collaborate with healthcare professionals to ensure comprehensive care, participate in on-call rotations, and maintain accurate records while upholding HIPAA regulations and patient privacy.

Qualifications

• Criminal background check required.
• Valid Oklahoma LPN license and CPR certification required.
• Driver's license required.
• Minimum 1 year of home health or hospital experience.
• HIPAA knowledge and confidentiality.
• Competency assessments completed.

Responsibilities

• Provide nursing care at patients' homes following a Plan of Care.
• Monitor patient condition and adjust care plan as needed.
• Assist with daily living activities (bathing, dressing, grooming).
• Administer medications and treatments as prescribed.
• Maintain accurate patient care records.
• Collaborate with healthcare professionals for comprehensive care.
• Participate in on-call rotation.
• Perform wound care and IV maintenance as needed.
• Identify patient needs and report to supervising nurse.
• Maintain professional attitude and promote teamwork with patients and staff.
