Job summary

TruHeart Home Health and Hospice in Tulsa, Oklahoma, seeks a PRN Licensed Practical Nurse (LPN) to provide in-home nursing care under a Plan of Care. This per diem role offers flexible scheduling and the opportunity to work closely with an RN/MD to support patient goals.

Qualifications

• Current Oklahoma LPN license.
• CPR certification.
• Driver's license and auto insurance.
• One or more years of experience in home health agency or hospital setting.
• Ability to work in a fast-paced environment with varying patient needs.
• Graduate of an accredited school of nursing for LPN.

Responsibilities

• Provide comprehensive health and nursing care in patients' homes.
• Administer medications and treatments per policy and plan of care.
• Perform baseline nursing skills as per agency policies.
• Assist with rehabilitation according to medical care plan.
• Identify patient needs and report abnormalities to supervisor.
