Job summary

Social Worker (MSW) PRN at Medical City Plano (part of HCA Healthcare) provides non-clinical biopsychosocial assessments and interventions for inpatients and ED patients. Leads discharge planning, family conferences, and resource referrals to support patient and family-centered care.

Role requires TX Licensed MSW, MSW degree, and 1+ year medical social work experience; certifications and LICWS preferred. PRN days with weekend coverage as needed.

Qualifications

• Master’s Degree in Social Work required.
• Currently licensed as a Registered Master of Social Work in the State of TX.
• Certification in Social Work preferred.
• Licensed Clinical Social Worker preferred.
• Minimum of 1+ years medical social work experience.

Responsibilities

• Collaborate with RN case managers, nurses, physicians, insurers, and staff to identify and address patient needs.
• Identify and address discharge barriers including financial, emotional, psychosocial, medical, and spiritual factors.
• Maintain knowledge of Medicare, Medicaid, and private payer regulations and managed-care concepts.
• Perform thorough assessments and develop discharge plans for high-risk patients with psychosocial needs.
• Coordinate and facilitate meetings between care teams and patients/families, supporting discharge planning and shared decision-making.
• Provide psychosocial support and advocacy for patients and families facing chronic or terminal illness, including crisis interventions.
• Offer emotional support to patients and families during end-of-life and other emotionally charged situations.
