Job summary

Cherish Hospice in Springfield, Ohio seeks a PRN Hospice Social Worker to provide flexible psychosocial support for patients, families, and caregivers at a profound life transition.

You will perform psychosocial assessments, participate in IDG meetings, monitor emotional well-being, and support family coping during end-of-life care, with travel to patient homes and facilities as needed.

Qualifications

• MSW required and licensed in Ohio.
• Minimum 1 year of hospice or palliative care experience preferred.
• Strong interpersonal and crisis intervention skills.

Responsibilities

• Conduct psychosocial assessments within 5 days of admission.
• Participate in IDG meetings and collaborate with the interdisciplinary team.
• Observe and report changes in emotional and social well-being.
• Assist patients and families in coping with the dying process.
• Evaluate effectiveness of social work interventions and adjust as needed.
• Participate in discharge planning as needed.
