Job summary

Otterbein SeniorLife is seeking a Hospice Social Worker in Ohio to provide comprehensive psychosocial support to terminally ill patients and their families. This PRN position involves assessments, counseling, care planning, and coordinating community resources within an interdisciplinary team.

The ideal candidate holds a Master’s degree in Social Work, an active Ohio license, and reliable transportation. Experience in hospice or home care is preferred but not required.

Qualifications

• Master’s degree in social work from a CSWE-accredited program.
• Active Ohio Social Work license and a valid driver’s license with reliable transportation.
• Minimum of 1 year experience in healthcare with terminal illness/family support preferred.

Responsibilities

• Perform psychosocial assessments for patients, families, and caregivers.
• Coordinate referrals to community resources and financial assistance as needed.
• Provide counseling on anticipatory grief and coping strategies.
• Participate in interdisciplinary team meetings to develop care plans.
• Document clinical information accurately and timely and assist with discharge planning.
