Job summary

Haven Home Health & Hospice seeks a compassionate PRN Hospice Medical Social Worker (MSW) to join an interdisciplinary team. You will provide emotional, psychosocial, and resource support to hospice patients and their families as they navigate end-of-life care.

Responsibilities include psychosocial assessments, counseling, connecting families with resources, and collaborating with nurses, physicians, chaplains, and aides.

Qualifications

• : Bachelor's or Master's Degree in Social Work or Sociology from an accredited institution.
• : Oklahoma Social Worker license or certification, as required.
• : One year of experience in hospice, home health, healthcare, or community-based services preferred.
• : Excellent communication and interpersonal skills.
• : Understanding of hospice philosophy and the end-of-life process.

Responsibilities

• : Complete psychosocial assessments and develop individualized plans of care.
• : Provide counseling and emotional support to patients, families, and caregivers.
• : Assist families with community resources, financial assistance programs, and advance care planning.
• : Collaborate with nurses, physicians, chaplains, aides, and the interdisciplinary team to provide holistic patient care.
• : Participate in weekly interdisciplinary team (IDT) meetings.
• : Maintain timely, accurate documentation in accordance with hospice regulations.
• : Support patients and families through grief, loss, and bereavement.
• : Assist with discharge planning and community resource referrals as appropriate.
• : Represent Haven professionally within the community and uphold our mission of compassionate hospice care.
