Job summary

Ohio’s Hospice in Wooster, OH is seeking a PRN Medical Social Worker to provide biopsychosocial assessments and plan of care for terminally ill patients and families. You will collaborate with the Interdisciplinary Team to coordinate services and document care in the electronic health record.

The role requires a Master's in Social Work, Ohio licensure (LSW/LISW preferred), and strong documentation and driving abilities.

Qualifications

• Licensed Social Worker (LSW or LISW) in Ohio.
• Master's Degree in Social Work required for licensure.
• Computer skills sufficient to document services and care.

Responsibilities

• Assesses, evaluates and documents biopsychosocial, safety, financial and bereavement needs.
• Develops and implements individualized plans of care with patient and family.
• Updates goals/interventions based on ongoing assessments and visit frequency.
• Participates in care conferences and interdisciplinary team meetings.
• Coordinates linkage of services with Ohio's Hospice and community providers.
• Documents patient and family demographics for Bereavement purposes.
