PRN Hospice Social Worker – Community-Based Care

Job summary

CareOregon, Inc. is seeking a PRN Hospice Social Worker to deliver psychosocial support to patients and families in the Portland metro area. The role requires 5 weekdays of availability per month and will be 100% community-based.

The position emphasizes interdisciplinary collaboration, bereavement planning, and coordination of care across care settings, with a strong focus on crisis management and advocacy to meet patient needs.

Qualifications

• Master’s degree in social work from an accredited college.
• Current Oregon LCSW, LMSW or CSWA license.
• Minimum one year hospice experience.
• Valid driver’s license, acceptable driving record, and automobile liability coverage or access to an insured vehicle.
• CPR certification at hire or within 6 months in position.

Responsibilities

• Perform initial and ongoing psychosocial/bereavement evaluations of patients/family/caregiver and assist in the development/implementation of the plan of care.
• Active participation in IDG meetings for development and evaluation of the patient’s plan of care.
• Coordinate plan of care and assist patient/family/caregiver in goals of care including Advance Directives and POLST forms.
• Provide counseling/education considering family dynamics and coping methods for all ages.
• Serve as liaison with hospitals and agencies to coordinate referrals and service continuum.
