Job summary

Suncrest Hospice seeks a Registered Nurse (RN) to provide compassionate hospice care on a PRN basis across the service area, including patients’ homes and care facilities.

You will join an interdisciplinary team, perform psych-social assessments, coordinate with social workers, chaplains, and care staff, and document care using an EMR. Ohio RN license and willingness to travel are required.

Qualifications

• Current Ohio RN license in good standing.
• Strong teamwork with interdisciplinary team members.
• Ability to travel across service area and work independently.
• Experience with hospice rules and patient privacy.
• Time management to schedule visits and document care.

Responsibilities

• Provide care and services for patients in homes and facilities.
• Visit patients across service area as needed.
• Collaborate with nurses, social workers, chaplains, and aides.
