Job summary

Suncrest Hospice is seeking a Registered Nurse to care for patients on a PRN (as needed) basis. You will visit patients in private homes, Skilled Nursing Facilities, and assisted living facilities, delivering compassionate hospice care.

You will work as part of an interdisciplinary team, performing psych-social assessments and coordinating with community resources to support patients and families. A valid Ohio RN license and reliable transportation are required to travel across the service area.

Qualifications

• Active Registered Nurse license in Ohio (in good standing).
• Ability to collaborate with nurses, social workers, chaplains, volunteers, and aides.
• Comfort with independent scheduling, phone/email communication, and meetings.

Responsibilities

• Provide care and services for a group of patients, visiting them where they reside (homes, SNFs, assisted living).
• Contribute to the interdisciplinary team with psych-social assessments and community resource coordination.
• Maintain documentation and coordinate visits, meetings, and urgent needs while triaging as necessary.
• Learn and use an electronic medical records system and ensure compliance with hospice regulations.
