Job summary

AdvisaCare Hospice and Palliative Care Program seeks a skilled Nurse Practitioner to join our team providing comprehensive in-home palliative care to elderly and frail patients.

Your role emphasizes collaboration with the Medical Director and interdisciplinary team to deliver high-quality patient-centered care in the home setting. A Michigan NP license and hospice experience are preferred, with opportunities for professional growth and ongoing education.

Qualifications

• Current unrestricted Nurse Practitioner in Michigan.
• Experience in Medicare-certified home health/hospice agency preferred.
• Knowledge of home health nursing/hospice and/or outpatient services preferred.
• Ability to meet the agency needs for Hospice Medical Director/designee.
• Understanding of performance improvement and ability to operationalize it.

Responsibilities

• Perform routine and urgent assessments in patients' homes.
• Order, perform, and interpret labs and imaging as appropriate.
• Prescribe medications including controlled substances within scope.
• Coordinate care with Medical Director and interdisciplinary team.
• Maintain accurate patient records and documentation for sound practice and reimbursement.
• Participate in Quality Assurance activities related to NP scope and policies.
• Educate patients and caregivers about care processes and options.
