Job summary

Empatia is seeking a compassionate Palliative Care Nurse Practitioner to deliver advanced palliative care at patients’ homes, collaborating with the Palliative Medical Director to guide care through all treatment stages.

The role requires an MSN, NP licensure, and at least two years of NP experience with family or adult medicine. Strong billing and documentation skills are essential for success in this safety-sensitive position.

Qualifications

• Master of Science in Nursing (MSN) or equivalent per state law.
• Minimum two years’ Nurse Practitioner experience.
• Specialty in Family or Adult Medicine preferred.
• Experience with Part B billing.

Responsibilities

• Collaborate with the Palliative Medical Director and care team to deliver comprehensive, patient-centered care.
• Perform in-depth patient assessments (physical, emotional, psychosocial).
• Develop and implement individualized care plans with patients and families.
• Prescribe medications (incl. controlled substances) and order tests as permitted by state law.
• Maintain timely clinical documentation and communicate with physicians and other healthcare professionals.
• Participate in evening/weekend call rotation as needed.
• Facilitate referrals for additional care services.
