Job summary

AdvisaCare Hospice and Palliative Care Program in Michigan seeks a Nurse Practitioner to join our compassionate team. You will perform assessments, manage patient plans, and collaborate with the Interdisciplinary Team to deliver high-quality hospice care in patients’ homes.

This PRN/Part Time role requires current Michigan NP licensure with preferred home health or hospice experience. You will advance care coordination, educate families, and uphold confidentiality and our mission.

Qualifications

• Current unrestricted Nurse Practitioner license in Michigan required.
• Minimum of one-year experience in Medicare-certified home health/hospice preferred.
• Knowledge of home health nursing/hospice and/or outpatient services preferred.
• Must meet agency need to support Hospice Medical Director/designee.
• Strong communication, organizational, confidentiality, and collaboration skills.
• Adheres to confidentiality and professional standards.

Responsibilities

• Perform routine and urgent assessments in patients' homes.
• Conduct medical evaluations for elderly, frail, seriously ill patients.
• Order, perform, and interpret labs and radiology tests within scope.
• Prescribe medications including controlled substances as licensed.
• Coordinate care and communicate goals with the Interdisciplinary Team.
• Maintain accurate patient records and support reimbursement.
• Participate in Quality Assurance activities related to NP scope.
• Provide staff training and onboarding support.
