Job summary

Empatia seeks a compassionate Palliative Care Nurse Practitioner to join our interdisciplinary team providing advanced home-based palliative care, collaborating with the Palliative Medical Director to guide patients and families toward quality of life. The role requires MSN, NP licensure, 2+ years in Family/Adult practice, and Part B billing experience.

Qualifications

• Master of Science in Nursing from an accredited school.
• Minimum two years of Nurse Practitioner experience.
• Family or Adult medicine specialty preferred.
• Experience with Part B billing and autonomous practice.
• Ability to work independently in an advanced role.

Responsibilities

• Deliver comprehensive, patient-centered palliative care in home settings.
• Perform in-depth patient assessments across physical, emotional, and psychosocial domains.
• Develop and implement individualized care plans with patients and families.
• Manage and adjust treatment plans based on progress and needs.
• Prescribe medications and order tests as permitted by state law.
• Maintain timely, accurate clinical documentation per standards.
• Communicate effectively with physicians, caregivers, and team members.
• Participate in evening/weekend call rotation as needed.
• Facilitate referrals for additional care services when appropriate.
