CommuniCare Family of Companies, owner of Dignity Care Hospice, seeks a dedicated RN Field Nurse to provide compassionate home-based care. Plan, organize and direct home services in patients’ homes, coordinating with LPNs and Home Health Aides to meet family needs.
Responsibilities include initial patient/family assessments, care planning, medication administration, supervising aides, discharge planning, and case management as assigned.
