Job summary

Dignity Care Partners Hospice IN in Indianapolis, IN is seeking a part-time Hospice Nurse Practitioner to provide high-quality end-of-life care to patients in home settings. The role involves collaboration with the Hospice Medical Director, health promotion, and chronic disease management.

You will perform face-to-face assessments, document findings, and ensure timely reports while maintaining compliance with regulatory standards. Reliable transportation and flexibility are essential.

Qualifications

  • Valid Nurse Practitioner’s license.
  • Master’s degree in nursing required.
  • Previous hospice and palliative care experience preferred.
  • Must have reliable transportation.
  • Daily home visits and weekly office meetings.

Responsibilities

  • Perform face-to-face patient encounters to determine continued eligibility for hospice care.
  • Document clinical findings and provide reports to the hospice physician within 24 hours of visits.
  • Maintain accurate patient records and support the interdisciplinary team.
  • Comply with legal, regulatory, and accreditation requirements.
  • Evaluate patients for appropriate level of care and participate in QAPI activities.
  • Support cultural practices and adhere to policies and procedures.
