Job summary

Dignity Care Partners Hospice IN in Indianapolis is seeking a compassionate Hospice Nurse Practitioner to deliver high-quality, patient-centered end-of-life care in home settings.

You will assess and manage patients, educate families, and collaborate with the Hospice Medical Director to ensure compliant, respectful care. This full-time role requires a current NP license in Indiana, a master’s in nursing, reliable transportation, and flexible availability.

Qualifications

  • Valid Nurse Practitioner license and current Indiana license.
  • Master's degree in nursing required.
  • Hospice/palliative care experience preferred.
  • Must have reliable transportation and valid driver's license.

Responsibilities

  • Provide health care to patients and education to families.
  • Perform face-to-face encounters to determine hospice eligibility.
  • Document clinical findings and maintain patient records.
  • Comply with legal, regulatory, and accreditation requirements.
  • Address and support cultural practices within care.
  • Participate in QAPI activities and in-service education.
  • May provide on-call support to staff.
  • Adhere to policies, procedures, and state/federal regulations.

A legacy that will make your life better as well.As a Hospice Nurse Practitioner, you will provide health care to patients and education to their families through the application of advanced knowledge and skills. In collaboration with the Hospice Medical Director, you will provide services related to health promotion, chronic disease management, and end of life care.Some of your primary responsibilities will include:Perform face-to-face encounters with the patient to determine continued eligibility for hospice care within time frame of regulatory guidelines.Document clinical findings in detail at patient bedside as related to the hospice criteria and provide a clinical report to the hospice physician within 24 hours of visit.Prepare and maintain accurate patient records and documentation to support interdisciplinary team and services provided.Comply with legal, regulatory, and accreditation requirements.Evaluate hospice patients for appropriate level of care.Assist in development of hospice protocols, policies, and procedures as required.Participate in QAPI activities as requested.Participate in in-service education related to areas of practice as required.Address and support cultural practices, as long as practices do not harm others.Adhere to all organization, payroll, billing, and documentation policies and procedures.Maintain knowledge of current federal, state, and accreditation standards and organization regulations concerning patient care.Adhere to MSA policies and procedures.Complete annual educational requirements.Ensure compliance with state and federal regulations, company policies/procedures, and process maps in the performance of job responsibilities.May provide on call support to staff.Contribute to team effort by accomplishing related results as needed.Benefits You Will EnjoyIn addition to competitive pay, full-time team member benefits include:Paid Time OffInsurance benefits (medical, dental, vision, life, and other voluntary group programs)401 (K) retirement savings plan with employer matchTuition reimbursement for qualified career pathsCareer advancement opportunitiesQualificationsValid Nurse Practitioner’s licenseMaster’s degree in nursingPrevious hospice and palliative care experience preferred.Must have reliable transportation.
