Job summary

EverHeart Hospice is seeking a Nurse Practitioner to support the Medical Director in guiding hospice and palliative care services. This role will directly care for patients and lead symptom management across home, inpatient, hospital, and nursing facility settings.

The NP will collaborate with physicians, care teams, and community providers to ensure compassionate, high-quality care and will help develop policies and protocols for palliative care within the agency.

Qualifications

• Master’s Degree in Nursing (MSN) from an accredited School of Nursing.

Responsibilities

• Provide palliative care consultation and symptom management with the attending physician.
• Coordinate with physicians, care teams, and community providers to ensure high-quality care across settings.
• Develop and update patient plans of care in collaboration with the Medical Director and interdisciplinary team.
• Provide clinical input, oversight, and medication management within licensure scope per hospice philosophy.
• Assist with developing and reviewing clinical protocols, policies, and procedures.
• Maintain knowledge of palliative care needs for patients with terminal illnesses.
• Complete documentation in compliance with agency policy and standards.
