Agape Care Group is seeking a Hospice and Palliative Care Nurse Practitioner to provide compassionate, patient-centered care to seriously ill patients in home and community settings.

The NP will relieve symptoms, collaborate with a multidisciplinary team, and educate patients and families about hospice and palliative options. We value experienced clinicians with a heart to serve and a commitment to excellence in care.

Qualifications

• Graduate of an accredited nursing program; NP credential required.
• Experience in chronic disease management or palliative/hospice care preferred.
• Must have reliable transportation and physical ability to assist patients as needed.

Responsibilities

• Care for seriously ill patients to alleviate burdensome symptoms.
• Coordinate with a multidisciplinary team to align treatments with patient goals.
• Provide hospice and palliative care and education to patients and families.
• Support patients and families in home or care settings.

As an active member of the medical community in your territory, you will provide both ongoing palliative care and hospice service and education.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing preferred
• Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively
