Job summary

ACG Hospice is seeking a dedicated Hospice Nurse Practitioner to deliver compassionate palliative and hospice care, guiding patients and families in their homes. You will collaborate with a cross-functional clinical team to ensure comfort and quality of life aligned with patient goals.

The role emphasizes education, pain and symptom management, and ongoing professional development within a supportive leadership structure. Opportunity for advancement is highlighted within our growing program.

Qualifications

• Heart to serve patients and families with high-quality care.
• Graduate of an accredited Nursing program (Certificate/Diploma/Associate/Baccalaureate).
• 2 years of chronic disease management/long-term care experience; hospice background preferred.

Responsibilities

• Provide palliative and hospice care in accordance with patient goals and values.
• Collaborate with a multidisciplinary team to deliver care in patients' homes.
• Educate patients and families on care plans and symptom management.
• Maintain reliable transportation and physical ability to lift as required.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
• Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively
