Job summary

CHRISTUS HomeCare in Lufkin, Texas is seeking a PRN/Per Diem Physical Therapist to join our dedicated team in home health. You will assess patients, develop therapy plans, and implement interventions to improve mobility and comfort, while collaborating with physicians, nurses, and caregivers.

The role offers flexible scheduling with requirements including state licensure, a valid driver's license, and CPR certification (preferred within 90 days).

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to a dependable vehicle, or public transportation.
• Preferred CPR certification (within 90 days of hire).

Responsibilities

• Evaluates patient functional status and PT needs; assists in developing therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient response to treatment and changes to supervisor/physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patients, family, and care team regarding plan of care and progress.
• Visits patients per Plan of Treatment and documents progress.
• Completes PT evaluations and develops plan of care per state guidelines.
• Reports outcomes, goals, and projection of care frequency.
