Job summary

CHRISTUS HomeCare in Lufkin, Texas is seeking a PRN / Per Diem Physical Therapist to join our patient‑focused home health team. You will perform assessments, develop treatment plans, and provide interventions to improve mobility and function in patients in their homes.

Primary responsibilities include evaluating functional status, coordinating with the care team, documenting progress notes, and delivering care within state guidelines.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice and coordinates with the health care team.
• Observes and reports patient response to treatment and changes in condition.
• Instructs patients and families about plan of care, functional limitations, and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes.
• Completes PT evaluations and develops the plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
