Job summary

CHRISTUS HomeCare in Texas is seeking a Physical Therapist to perform assessments of functional status, mobility, endurance, and pain in home health. You will determine a treatment plan, implement interventions, and monitor progress to improve patient well-being.

This role requires state licensure, a valid driver’s license, and access to reliable transportation. CPR certification is preferred; the organization emphasizes caring and teamwork across the home health network.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and reliable vehicle or access to transportation.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs. Assists the physician in developing the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and in coordination with the health care team.
• Observes and reports patient responses to treatment and any changes to supervising staff.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs patients, family, and care team on the plan of care and progress toward goals.
• Visits patients according to Plan of Treatment and documents progress notes.
• Completes evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and care frequency.
• Functionally adapt to patients’ diverse backgrounds and conditions.
