Job summary

Venture I Inc in Lufkin, TX is seeking a licensed Physical Therapist to join our Home Health & Hospice team. The PT will evaluate patients, create individualized treatment plans, and deliver therapy in patients' homes.

You will collaborate with physicians and the care team, educate patients and caregivers, document in the EMR, and help maintain compliance with Medicare and HIPAA. Part-time or PRN schedule with travel to service areas.

Qualifications

• Graduate of CAPTE-accredited Physical Therapy program.
• Bachelor's, Master's, or Doctor of Physical Therapy (DPT) degree required.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide skilled PT services in home health and hospice care.
• Educate patients and caregivers on exercises, safety, and disease management.
• Complete EMR documentation accurately and timely.
• Collaborate with physicians and interdisciplinary team members.
