Qualifications

• Home care experience is desired.

• Valid driver's license and auto insurance.

Responsibilities

• Provide skilled physical therapy in patients' homes as ordered by the physician.
• Observe and report patient status changes to physician and care staff.
• Perform OASIS assessments to guide care planning.
• Develop and adjust plan of care.
• Educate patient and family on therapy and use of equipment.
• Ensure proper documentation and compliance with Medicare guidelines.
