Job summary

Alternate Solutions Health Network seeks a licensed Physical Therapist to provide skilled therapy in patients' homes under physician orders. You will assess needs, develop and adjust plans of care, and coordinate with the care team to promote recovery and prevent hospitalizations.

The role requires traveling to patient locations in the Hamilton area and surrounding regions, documenting care, and ensuring compliance with Medicare guidelines.

Qualifications

• Minimum one year of PT experience in an acute setting.
• Home care experience desired.
• Ability to communicate effectively with patients, families, physicians and coworkers.
• Knowledgeable on therapy best practices and clinical judgment.
• Detail oriented with disciplined work ethic and timeliness.
• Valid driver's license and auto insurance in your name.

Responsibilities

• Provides skilled physical therapy in a patient's home as ordered by the attending physician.
• Observes, records, and reports patient reactions, changes, and progress to attending physician.
• Performs OASIS assessments to develop a plan of care and adjust as needed.
• Develops plan of care and treats patients to relieve pain and restore function.
• Drives to patient's primary location per scheduled visit and documents all care.
• Promotes continuity of care with admissions, transfers and discharges.
