Job summary

Alternate Solutions Health Network in Ohio is seeking a Physical Therapist to provide comprehensive home-based therapy services. You will work with an interdisciplinary team, educating patients and families while delivering compassionate care in patients’ homes.

Schedule: Sat/Tues/Thurs, 8:30am-5pm; 8 PIQs daily; on-call weekends per rotation and one holiday per year. Base salary with uncapped bonus, mileage reimbursement, and a competitive benefits package.

Qualifications

• Licensed Physical Therapist in the state of employment (OH).
• Minimum one year PT experience; acute care experience preferred.
• Home health experience encouraged.

Responsibilities

• Provide skilled home-based PT services as ordered by physician.
• Educate patients and families; ensure safety and documentation.
• Perform OASIS assessments and develop an individualized plan of care.
• Supervise PTAs and nurse aides as applicable.
• Maintain compliance with Medicare/Federal guidelines and agency policies.
