Job summary

Elite Home Health Group INC is seeking a PRN Licensed Occupational Therapist (OT) to provide skilled home health services in Spring Hill, FL. You will evaluate patients, develop individualized plans, and help them regain independence in daily activities, safety, and mobility within home settings.

Qualifications

• Graduate of an accredited OT program.
• Active Florida OT license (OTR).
• Minimum 1 year OT experience.
• CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Perform initial and ongoing occupational therapy evaluations to assess patients' functional abilities and therapy needs.
• Develop, implement, and update individualized treatment plans in accordance with physician orders.
• Provide skilled therapy interventions to improve daily living skills, functional mobility, safety, and independence.
• Educate patients and caregivers on home safety, adaptive techniques, self-care strategies, and therapeutic activities.
• Assess home environments and recommend appropriate modifications or adaptive equipment.
• Monitor patient progress and adjust treatment interventions as needed.
• Complete accurate and timely clinical documentation, progress notes, reassessments, and discharge summaries.
• Collaborate with physicians, nurses, and other team members.
• Supervise Occupational Therapy Assistants (OTAs) and Home Health Aides as required by regulations and policy.
• Maintain compliance with Medicare requirements and agency policies.
