The Pennant Group is hiring a Home Health Licensed Practical Nurse in Kent, WA. You will provide direct patient care under RN supervision, following the plan of care and delivering treatments to help achieve positive patient outcomes.

Requirements include graduation from an accredited practical nurse program, 1–2 years of nursing experience preferred, LPN/LVN licensure in the practice state, CPR certification, and a valid driver’s license with an insured vehicle.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• One to two years nursing experience preferred.
• Currently licensed as an LPN/LVN in the state(s) of planned practice.
• Must be a licensed driver with an insured vehicle.
• Possesses CPR certification.
• Geriatric population knowledge and common needs.

Responsibilities

• Provide direct patient care under the supervision of a registered nurse.
• Follow the plan of care and provide treatments in collaboration with the team.
• Document patient services accurately to reflect the plan of care.
• Educate patient and family/caregivers pertinent to diagnosis and plan of care.
• Use and prepare equipment and supplies effectively; manage pain and symptoms.
• Report significant changes in patient condition to the RN Case Manager.

Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.Provides direct patient care as defined in State Nurse Practice Act. Implements current nursing practice following a comprehensive assessment and the plan of care.Provides accurate and timely documentation of patient services to reflect the plan of care.Evaluates need for and provides patient and family/caregiver education and information pertinent to diagnosis and individualized plan of care.Uses and prepares equipment and supplies effectively and efficiently.Provides appropriate pain/symptom management.
