Qualifications

• 1+ year of nursing experience.

Responsibilities

• Provide direct patient care in home health settings.
• Implement plan of care initiated by the RN.
• Document accurately and timely per plan of care.
• Collect assessment data and report findings to RN/physician.
• Coordinate home health services with the RN and Clinical Director.
• Use equipment safely and efficiently.
