Job summary

Signature Healthcare at Home is seeking a Licensed Practical Nurse to cover South Whidbey Island for home health skilled nursing services. This role follows a Monday–Friday day shift with self-directed scheduling to meet patient needs across Whidbey Island communities.

You will provide direct patient care per the plan of care, collaborate with the RN Case Manager, and ensure timely documentation. On-call duties and potential cross-area coverage are included.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years acute care in an institutional setting; 1–2 years home health preferred.
• Currently licensed as a Practical Nurse in Washington State.
• CPR certification required; valid driver’s license and vehicle access.

Responsibilities

• Provide direct patient care per the plan of care (wound care, catheterization, education, med management).
• Collaborate with RN Case Manager to develop the plan of care with patient’s provider.
• Complete documentation promptly and accurately in the chart.
• May be required to work outside primary area due to staffing needs.
• Participate in on-call duties (3–4 weekdays and 1–2 weekend days per month).

Signature Healthcare at Home is seeking to add a Licensed Practical Nurse for South Whidbey Island!This position would cover Whidbey Island (Greenbank, Clinton, Columbia Beach, Coupeville) for home health skilled nursing services.This position works Monday through Friday day shift, with self-directed scheduling dependent on patient needs and territory. If you enjoy a high level of autonomy, travel and meeting patients where they are, apply today!Duties and ResponsibilitiesProvides direct patient care according to the plan of care, such as but not limited to wound care, catheterization, disease process education, medication management and education.Works with the Registered Nurse Case Manager to develop the plan of care in partnership with the patient's primary care provider.Completes documentation timely, evaluates patient's needs and records pertinent information into the chart.May be required to work outside of the primary assigned area due to staffing and census fluctuations.Participates in on-call duties and takes assigned on-call shifts throughout the month, usually 3-4 weekdays per month and 1-2 weekend days per month.Knowledge, Skills and ExperienceGraduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting, and one (1) to two (2) years of recent experience in home health preferred.Currently licensed as a Practical Nurse (LPN) in Washington State.Possesses and maintains CPR certification.Must be licensed and able to practice in Washington State.The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job.
