Job summary

Signature Home Health in Oregon is seeking a Home Health LPN to join our compassionate team. This role offers full-time or part-time opportunities and focuses on delivering direct patient care in home health settings, following plans of care created by registered nurses.

You will document meticulously and collaborate with RNs and physicians to ensure optimal patient outcomes. Applicants should hold LPN licensure in Oregon, have at least 1 year of nursing experience, and maintain a current BLS

Qualifications

• Licensed Practical Nurse (LPN) licensure in Oregon.
• 1+ year of nursing experience.
• Hold a current BLS certification from AHA.
• Ability to work independently or in a team.

_ Compensation is commensurate with experience starting at $42/hr. _

What You’ll Do:

• Provides direct patient care.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Collects assessment data and performs observation to report to and collaborate with RN(s) and physician(s).
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nurse and/or Director of Clinical Services.
• Uses equipment and supplies effectively and efficiently.
• Participates in personal and professional growth and development.
