Signature Healthcare at Home is seeking a Home Health LPN to join our after-hours on-call team. You will respond to triage calls, complete patient visits, and perform follow-ups with patients and caregivers in home settings.

Ideal candidates will be an LPN in good standing with CPR certification, possess strong communication skills, and work autonomously with minimal supervision. Prior hospice or home health experience is preferred.

• Hold an active LPN license in the state of Oregon (as specified in the posting).

What You’ll Do:

This position is an LPN who works outside of scheduled business hours. This person is responsible for being the contact for Triage nursing and responds to calls as they come in while on shift. This position is responsible for completing patient visits and any required follow-up for patient/caregiver needs as well as any scheduled visits after business hours. The nurse builds from the resources of the community and collaboration with the Interdisciplinary Group/Team to meet the holistic needs of individuals and families within their homes and communities with an emphasis on palliative care for hospice patients and rehabilitative care for Home Health patients.

Essential Job Duties:

1. Provides nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act and within the scope of LPN/LVN duties.
2. Assesses and evaluates patient’s status by evaluating patient and family/caregiver needs as directed and participating in revising the plan of care in collaboration with the RN, the attending physician (or approved medical provider), and /or the hospice medical director as the patient status and needs change.
3. Works autonomously within Scope of Practice to initiate appropriate preventive rehabilitative and/or palliative care nursing procedures.
4. Administers medications and treatments as prescribed by the physician (or approved medical provider) in the patients’ plan of care.
5. Involves the patient and family/caregiver to address needs and meet related goals.
6. Provides health care instructions to the patient as appropriate per assessment and plan.
7. Assists in facilitating the patient’s efforts toward self-sufficiency or obtaining appropriate caregiving and optimal comfort care or independent level of functioning.
8. Documents patient services and status timely and accurately within professional practice standards and reimbursement requirements.
9. Works in cooperation with the family/caregiver and hospice interdisciplinary group home health interdisciplinary team to meet the emotional needs of the patient and family/caregiver.
10. Attends required staff meetings and interdisciplinary group meetings or case conferences.
11. Supports the mission, values, and goals of Signature Healthcare at Home.
12. Ensure that arrangements for equipment and other necessary items and services are available.

What we’re looking for:

• Graduate of an accredited school of nursing (Required).
• 1 - 2 years of recent acute care experience in an institutional setting (Preferred).
• 1 -2 years of recent experience in hospice or home health (Preferred).
• Currently licensed as a Licensed Practical Nurse in the State of Oregon (Required)
• Possesses and maintains CPR certification (Required).
• Communicate effectively, both written and orally, with all levels of the organization (Required).
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
• Self-directed and able to work with minimal supervision.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements.
