Job summary

Extended Family Home Care Services is seeking PRN CNAs and Home Health Aides to provide in-home patient care in Southwest Florida. Eligible staff can participate in the 401(k) after hire, with hours that support a flexible schedule and ongoing training.

A strong culture and competitive pay are emphasized in an expanding agency. Responsibilities include assisting with personal care, following a nurse-directed care plan, and coordinating visits to ensure safe, compassionate home care for clients

Qualifications

• High school diploma or equivalent.
• Certified Nursing Assistant or Certified Home Health Aide.
• Active AHCA Fingerprints (Level II Clearance) with Eligible status.
• CPR/First Aid Card by American Heart Association, American Red Cross, or the National Safety Council.
• Recent TB Test Result (within 12 months) or Chest X-Ray (within 5 years).
• State of Florida Driver's License with insured vehicle.
• The ability to drive and access clients’ homes, which may not be wheelchair accessible.
• Hearing, eyesight, and physical dexterity to perform care.
• Must be able to communicate in English clearly, both verbally and in writing.
• Must be free from health problems that may be injurious to client, self and co-workers.
• Must comprehend the basics of personal care, housekeeping and meal preparation.
• Must understand and respect client’s including ethics and confidentiality of care.

Responsibilities

• Provides personal care and related services in the home under supervision of the Supervising Nurse.
• Tasks to be performed by an HHA must be assigned by and performed under RN supervision.
• Follows the plan of care to provide safe, competent care/service.
• Helps the client maintain personal hygiene and a safe environment.
• Plans and prepares nutritious meals.
• Assists the client with ambulation as approved.
• Encourages client independence according to the care plan.
• Reports changes in client condition to the supervisor.
• Performs routine housekeeping tasks related to client safety.
• Prepares a visit report weekly and documents in the client record.
• Coordinates scheduling of visits to optimize staff coverage.
• Works with community agencies involved in client care.
• Attends required in-service trainings.
• Communicates with supervisor about client status and planning of service.

Education

Certified Nursing Assistant

Active AHCA Fingerprints (Level II Clearance)

CPR/First Aid Certification

State of Florida Driver's License
