Job summary

Extended Family Home Care Services, LLC is hiring PRN CNAs and Home Health Aides to provide in-home care across Southwest Florida. You’ll work under a supervising RN, delivering compassionate care and following a detailed care plan.

Qualifications

• High school diploma or equivalent required.
• Certified Nursing Assistant (CNA) or Certified Home Health Aide preferred/required.
• Active AHCA Fingerprints (Level II) clearance.
• CPR/First Aid Card required from recognized authorities.
• Recent TB test or Chest X-Ray within specified window.
• Florida driver’s license and insured vehicle are required.

Responsibilities

• Provide personal care and related services in the home under supervision.
• Follow care plan and RN supervision for client care and documentation.
• Assist with daily activities, hygiene, meals, mobility, and safety.
• Maintain cleanliness and safe home environment; perform light housekeeping.
• Prepare visit reports; coordinate weekly scheduling with supervisor.
• Communicate changes in client condition to supervisor and care team.
