Job summary

Majestic Care of Terre Haute is seeking a Physical Therapist to evaluate and treat residents in a skilled nursing facility, focusing on mobility, strength, endurance, and independence. You will collaborate with an interdisciplinary team to deliver resident-centered care, including education for residents and families.

The role requires current state licensure and a commitment to geriatric rehabilitation, safety, and regulatory compliance.

Qualifications

• Current state licensure as a Physical Therapist
• Experience in geriatric rehabilitation preferred

Responsibilities

• Conduct initial and ongoing evaluations of residents' mobility, strength, balance, and functional limitations
• Develop and implement individualized treatment plans based on clinical findings and resident goals
• Provide direct PT interventions including therapeutic exercises, gait and balance training, mobility enhancement, and manual therapy
• Use evidence-based practices to promote functional outcomes and independence
• Monitor progress and adjust plans as residents' conditions change
• Educate residents, families, and caregivers on treatment plans, home exercises, and safety strategies
• Collaborate with nursing, OT, and SLP teams for coordinated, resident-centered care
• Maintain accurate documentation of evaluations, treatments, progress notes, and discharge planning according to regulations
• Adhere to HIPAA, OSHA, infection control, and Medicare PDPM requirements
• Stay current with PT developments and provide training to team members
