Job summary

Majestic Care of Fairfield is seeking a licensed Physical Therapist to assess residents in a skilled nursing facility and design individualized treatments that improve mobility, balance, and independence. You will work with a multidisciplinary team to deliver compassionate, patient-centered rehabilitation for older adults.

Qualifications

• Current state licensure as a Physical Therapist.

Responsibilities

• Conduct initial and ongoing evaluations to assess residents’ physical therapy needs.
• Develop and implement individualized treatment plans based on clinical findings, resident goals, and functional abilities.
• Provide direct physical therapy interventions such as therapeutic exercises, gait and balance training, mobility enhancement, and manual therapy.
• Utilize evidence-based practices to promote optimal functional outcomes and independence.
• Monitor and document residents’ progress, adjusting treatment plans as necessary to reflect changes in condition or goals.
• Educate residents, families, and caregivers on treatment plans, home exercises, and safety strategies to support continued progress and prevent injury.
• Collaborate with interdisciplinary team members—including nursing, occupational therapy, and speech therapy—to ensure coordinated, resident-centered care.
• Oversee and support physical therapy assistants (PTAs) in accordance with state practice acts and facility guidelines.
• Provide coaching, development, and corrective action as needed to ensure high-quality service delivery.
• Evaluate team member performance and offer constructive feedback to promote professional growth and accountability.
