Job summary

North Houston Transitional Care is seeking a PRN CNA for 12-hour healthcare shifts. The role provides routine nursing care per care plans, with emphasis on resident safety, privacy, and comfort. You will identify residents, document care, and report changes to the Nurse Supervisor.

Responsibilities include assisting with daily living activities, preparing residents for tests, and supporting the nursing team during shifts. CPR preferred and state CNA license required.

Qualifications

• Must be a licensed CNA in accordance with state laws.
• CPR certification preferred.

Responsibilities

• Provide routine daily nursing care as per resident care plan.
• Identify residents before procedures and document accurately.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse.
• Maintain confidentiality of resident information and ensure security of electronic records.
• Assist residents with daily functions and mobility as trained.
• Prepare residents for medical tests and accompany them as needed.
• Participate in nursing reports and follow policies and procedures.
