The position requires a BS or PharmD, 1 year of pharmacy experience, and relevant licenses/certifications (TX licensure within 30 days, ACPE-IV, BLS; ACLS for inpatient roles

Qualifications

• Education BS or PharmD degree.
• 1 year of pharmacy experience; English proficiency.
• Eligible for Texas State Board of Pharmacy licensure on hire; TX license within 30 days.
• ACPE-IV Sterile Compounding certification; BLS required; ACLS required for inpatient positions within 180 days.

Responsibilities

• Performs job responsibilities in accordance with organizational policies and compliance standards.
• Reviews computer orders and updates medication profiles with accuracy and timeliness.
• Reviews medication orders for therapeutic appropriateness, interactions, and safety; documents interventions.
• Supervises technicians and staff; ensures proper handling of controlled substances; audits usage.
• Communicates drug information to physicians and nurses; educates patients and families as needed.
• Supports P&T initiatives; monitors target drugs and dosage safety; contributes to cost-effective pharmacotherapy.
• Duties may include direct patient interactions and maintaining patient records.
