Prevea Health is seeking an Advanced Practice Provider (NP or PA) to deliver care to patients under physician supervision. You will assess, manage, and coordinate care, including follow-up exams and counseling to promote health and prevent disease.

Qualifications

• Graduate of a master's level nurse practitioner program OR graduate from an accredited physician assistant program. Required.

Responsibilities

• Takes assessment, evaluation, and management of patients with minor, acute, chronic, and episodic illness or injury.
• Collaborates with physicians and other health professionals to develop care plans and coordinate patient care.
• Provides case management and refers patients to needed services.
• Reports test results and provides health counseling and education to patients and families.
• Orders diagnostic tests and medications within scope and maintains electronic medical records.
• Maintains patient records and collaborates to ensure optimal outcomes.
