Job summary

Prevea Health in Sheboygan, Wisconsin is seeking a PRN Advanced Practice Provider (NP or PA) to deliver high quality care under physician supervision. This role focuses on health promotion, disease prevention, and collaboration with a team to coordinate patient care.

You will assess and manage patients across ages, perform follow-up exams, order tests, prescribe medications as appropriate, and document in the electronic medical record. Prior NP/PA experience and Wisconsin licensure are required.

Qualifications

• Graduate of a master's level NP program OR graduate from an accredited physician assistant program. Required
• 1-3 years work experience in a clinical setting preferred
• Licensure as Nurse Practitioner or Physician Assistant in Wisconsin and related certification required
• CPR/BLS for Healthcare Providers certification required

Responsibilities

• Provides assessment, evaluation, and management to patients with minor, acute, chronic, & episodic illness & injuries.
• Collaborates with patients to review health background and determine needs, advising on future health plans.
• Provides case management services to assist clients and families with referrals to needed services.
• Conducts follow-up calls and provides health counseling and education to patients and families.
• Orders diagnostic tests, prescribes pharmacological agents, and other treatments as appropriate.
• Maintains and reviews patient records in the electronic medical record.
• Collaborates with physicians and other providers to develop and implement patient care plans.
