Job summary

Legacy Healthcare Services, Inc. is seeking a dedicated Physical Therapist to deliver individualized rehabilitation to senior residents, collaborating with a multidisciplinary team to plan and adjust treatment as needed.

You will conduct one-on-one therapy sessions, apply function-based approaches, and contribute to high-quality care that improves residents’ quality of life within a not-for-profit senior living network.

Qualifications

• Degree in area of discipline.
• Current Physical Therapy license in state of practice, or ability to obtain prior to starting.
• Excellent interpersonal, fiscal, and clinical skills.
• Computer proficiency.

Responsibilities

• Provide rehabilitative services to patients under specific medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to share and coordinate the plan of care for patients.
• Treat residents one on one, not in a group setting, so each patient gets individualized attention through private treatment sessions with a functional based treatment approach.
• You have the ability to individualize treatment sessions.
