Legacy Healthcare Services, Inc. is seeking a Physical Therapist to deliver individualized rehabilitation for senior residents within a dedicated team. The role emphasizes relationship-building, learning new care delivery methods, and contributing to patients’ quality of life.

You will plan and execute treatment programs based on clinical findings, attend conferences, and provide one-on-one sessions to ensure personalized care.

Qualifications

• Degree in area of discipline.
• Current Physical Therapy license in state of practice, or ability to obtain prior to starting employment.
• Excellent interpersonal, fiscal, and clinical skills.
• Computer proficiency.

Responsibilities

• Provide rehabilitative services to patients under specific medical orders as part of a coordinated team.
• Plan initial and subsequent treatment programs based on clinical findings.
• Participate in clinical conferences to share and coordinate the plan of care for patients.
• Treat residents one on one, not in a group setting, ensuring individualized attention.
• You have the ability to individualize treatment sessions!
